Premier licensed Home Care Agency is seeking a full time Client Care Coordinator (RN) to perform initial assessments and routine reassessments for our Home Care clients in the community. Must be a NYS licensed professional with at least 1 yr. experience in the health/home care field and possess good communication skills.
Job Summary:

The Client Care Coordinator will perform client assessments and develop an appropriate plan of care for each client. Duties will also include Home Health Aide supervision, training and ensuring adherence to the client plan of care to ensure quality of service. The Client Care Coordinator will work under the direction of the Director of Clinical Services.

Qualifications:

The Client Care Coordinator will be a licensed nursing professional within the State of New York with at least one years experience in the home health care field. They must possess a current driver’s license with valid auto insurance and demonstrate excellent communication skills a positive professional business image at all times. The Client Care Coordinator will also participate in a regular on-call rotation. Knowledge of the Department of Health regulations is helpful but not a requirement.
